Reducing Readmissions Using a Team Approach | July 30
Greenwood County Hospital is a 25-bed Critical Access Hospital in Eureka, Kansas with a population of 2,332 residents. In 2021, readmission rates continued to be high despite previous improvement efforts, indicating a need to re-evaluate process and strategies. The Readmissions Team was restructured at that time, adding team members and returning to basic best practice interventions. Collaboration continues to expand with care team members outside of the hospital, including home health, primary care and chronic care management services. Since restructuring, readmission rates have decreased by 33%.
